To apply for a 2010-11 membership in the PTG:

  1. Download a membership application form
  2. Print the form.
  3. Complete the form
  4. If you are paying with Visa or MasterCard and wish to have the charge slip sent to you, include a self-addressed, stamped envelope with your application
  5. Mail the completed form to:
